Four men and a company have been charged under the Safety, Health and Welfare at Work Act in relation to the death of John Knight in an industrial incident on March 6, 2006 at Forest Park, Mullingar Industrial Estate.
John Hunt, Tommy Kelly, Eugene Shiel, and Kieran Carroll, and the company with which they are involved, Technical Engineering and Tooling Services, are to appear in court on February 5.
They face a number of different charges in relation to the incident, including failure to maintain machinery, failure to adequately manage and conduct work activities, and failure to provide training and instruction.
The charges relate to a piece of equipment called a Hurco VMX30S, on which it is alleged the company failed to maintain a safety interlocking guard.
