The Integrated Discharge Planning brochure, which was developed by staff from Mayo General Hospital and the HSE’s Primary, Community and Continuing Care services in Mayo, is aimed at making the discharge process for patients a seamless transition from one stage of care to the next, and is part of the HSE’s national Code of Practice for Integrated Discharge Planning.
The leaflet covers areas such as the patient’s discharge plan, medication, the day a patient goes home and things to do before the patient goes home, as a key target of the hospital-to-home journey is effective discharge planning to facilitate patients leaving the hospital by 11am on the day of discharge. The patient information leaflet will be available at ward level for patients and families.
